The My Hospital Pack is an easy-to-use envelope template (interactive and standard) with two checklists: 

  • Ready for Hospital - to be completed in the community (for example: at home)
  • Ready for Home - to be completed by hospital staff as a discharge tool

The My Hospital Pack can be used to prepare and store essential information in case of a hospital stay – planned or unplanned.
