RC questions SA Chief Executive on incident response

The Chief Executive of the SA Department of Health was asked for her response to the cases examined during the hearing. Questions touched on the stateâs case management approach, formal feedback processes and transparency.
Senior Counsel explored the adequacy of internal audit reports related to residences considered this week. The witness said significant work had been undertaken since the auditor's findings that no formal processes were in place for families to raise concerns. This had been rated a 'minor' risk, which she agreed was too low.
