Who is this for?

This catalogue has been developed for health professionals and those that support people with disability transition between hospital and community as well as people with disability within Western Australia.

How do I use the catalogue?

This catalogue has been produced as part of the Ready to Go Home project. The RTGH project aims to reduce hospital discharge delays for people with disability in WA and improve the transitions for people with disability from community to hospital and hospital to community settings.

The resource catalogue has been organised by audience and can be searched by focus area and resource type.
