RC hears of systemic failures and cultural issues

Concerns about culture and a number of systemic issues were the focus of todayâs evidence on the running of a Sunnyfield group home, the subject of this weekâs case study. Detail was provided on investigations into allegations of violence, and investigator findings and recommendations.
The provider engaged an investigator to interrogate multiple allegations against two workers, referred to as âSP1â and âSP2â. Where scope allowed, she made findings about the likelihood of allegations being sustained on the balance of probability, and recommendations. Balance of probability is a lower threshold than required for criminal investigations.
