In the morning, the Royal Commission put the internal investigation function of the SA Department of Human Services under the spotlight. The manager of the area has a dual reporting function – to the CEO of the Department and to the Chair of the Audit Committee – to help ensure its independence.

Internal audits of one of the homes discussed over the past two days confirmed it operated with 'an air of neglect', smelt of urine, and did not provide a home-like environment. Despite funding approved for cleaning the carpets, the work remained undone for over two years. In addition, internal audits demonstrated significant deficiencies in the two investigations into the threats made against Mitchell. Questioning by Counsel elicited responses that differed from the written statement provided.
