  • Ready to go home resources have been designed to reduce hospital discharge delays for people with disability in Western Australia and improve the transition between hospital and community settings.
  • These guides will:
    • assist people with disability with preparing and being informed for a hospital stay
    • assist people with disability that are new to the NDIS and may need to consider applying for the NDIS while in hospital.
  • Guides can also be used by health service provider staff and services provider support staff.

Who is this for?

These resources have been developed for people with disability transitioning from hospital to community settings, those preparing to access the National Disability Insurance Scheme and support staff. Easy read guides have also been developed for people with cognitive and/or intellectual disability.

Your Hospital Guides

Ready to Go Home (RTGH) has produced the Your Hospital Guide series to assist with preparing and being informed for a hospital stay in WA.

The series comprises seven guides:

  1. Be prepared for a hospital stay
  2. Your hospital stay
  3. Decision-making during a hospital stay
  4. Discharge planning checklist
  5. Your discharge summary
  6. After your hospital stay
  7. NDIS basics
  • Your Hospital Guides 1 to 6 have been designed for people with disability.
  • Your Hospital Guide 7 is designed for people with disability preparing to access the NDIS.
  • Your Hospital Guides 1 to 7 can also be used by health service provider staff and services provider support staff.
  • Your Hospital Guides 1 to 7 Easy Read are for people with cognitive and or intellectual disability.

The series has been co-designed by people with lived experience of disability and hospital stays.
