Ready to Go Home Resources

The Ready to Go Home project resources have been co-designed and co-developed with health consumers with disability, disability service providers and health professionals to:
- improve the hospital experience for a person with disability, and
- reduce barriers that contribute to discharge delays
Who is this for?
- Your Hospital Guides are for people with disability and their support networks
- My Hospital Pack is for people with disability, their support networks, health service providers, and disability service providers
- Service Provider resources are for disability services and mental health organisations
- Health professional resources are for clinicians and hospital service providers
Your Hospital Guides
Your Hospital Guides 1-7 are available in booklet, easy print, Easy Read and Accessible Word versions:

The My Hospital Pack is an easy-to-use envelope template (interactive and standard) with two checklists:
- Ready for Hospital - to be completed in the community (for example: at home)
- Ready for Home - to be completed by hospital staff as a discharge tool
The My Hospital Pack can be used to prepare and store essential information in case of a hospital stay – planned or unplanned.
