North metro-based services providing home and living supports are invited to an event to develop an information transfer solution for hospital admissions and discharges for people with disability.

Hosted by Joondalup Health Campus and the Ready to Go Home project, the Disability Health Connect event aims to:

  • Identify information critical to aid smooth transitions for people with disability
  • Explore solutions to enable timely consent to share information 
  • Design an effective mechanism for the transfer of patient information

The Ready to Go Home (RTGH) project is working with Joondalup Health Campus on initiatives to improve discharge planning processes for people with disability and the hospital stay and transition experience for people with disability.

The Joondalup Health Campus RTGH pilot was initiated in February 2022 and will run for 12 months to February 2023.

The Ready To Go Home project is funded by Department for Social Services­—Information Linkages and Capacity Building initiative.
