Do you know how to document in a way that will protect you if your care is questioned?

This course will simplify the way you document and give you the confidence to document the support you provide in a way which is accurate, legally defensible, provides evidence of person-centred care and improves outcomes for staff, your service, and the people you support.

Course outcomes

Part 1: E-Learning (self-paced online learning module)

Duration: 45 minutes
Date: Elearning available from Monday 6 February 2023, to be completed before the workshop (Part 2)
Location: Online

Once you have completed the elearning (Part 1), you will receive the zoom link to the workshop (Part 2).

• Learn how to write progress and case notes which are clear and concise
• Be aware of the importance of using factual objective language and learn how to do it
• Understand what legibility means and why it is important
• Learn what is important to include (and what not to include) to ensure your notes are defensible and provide evidence of your Duty of Care

Part 2: Instructor Led Training (ILT) via Zoom

Duration: 1.5 hours
Date: Wednesday, 15 February 2023
Time: 1:00 pm to 2.30 pm AEDT 
Location: Online

Pre-requisite: Must have completed Part 1 E-Learning module

• Learn the FACT® method and how to apply it to your documentation
• Understand how to document evidence of working in partnership with the person you support
• Practice writing documentation using Case Studies
• Be aware of what should be included when documenting goals, risks, incidents and complaints

Part 3: E-Learning (self-paced online learning module)

Duration: 15 minutes

Pre-requisite: Must have completed Parts 1 and 2

• Consolidate your understanding by completing a note using the FACT® method

This workshop is in partnership with Preventing Harm Initiative (PHI) and facilitated by Donny Raets. Donny's expertise is in creating manageable ways to identify, report, investigate and most importantly, how to prevent or reduce the likelihood of recurrence. 

Donny holds a Diploma of Teaching, Cert IV Training and Assessment, Cert IV Aged and Community Care and has 30 plus years’ experience in education, ranging from Primary through to Tertiary (Vocational Education and Training), remote Indigenous Communities and the Corporate sector. Her CALD background and training in the Arts support her creation and delivery of meaningful learning experiences.
